You will need to provide us with the following information:

Your Full Name.
Address.
User or Matter Reference Number.
Proof of your identity (such as a copy of your driver’s license or passport).
Proof of your address (such as a recent utility or credit card bill).
A description of what right you want to exercise.
The information to which your request relates.

We are not obligated to disclose any information if we cannot verify that the person making the request is the person about whom we collected information or is someone authorized to act on such person’s behalf.

Any personal information we collect from you to verify your identity in connection with your request will be used solely for verification purposes.

Please Note:

California Users: You can only make a request related to the California Consumer Privacy Act (CCPA)/California Privacy Rights Act (CPRA) twice within a 12-month period. Both requests will be free of charge. We are not obligated to process your request more than twice within a 12-month period.
Virginia Users: You can only make a request related to the Virginia Consumer Data Protection Act (VCDPA) twice in a 12-month period. Both requests will be free of charge. In cases, however, where the requests are manifestly unfounded, excessive, or repetitive, we may charge a reasonable fee to cover the administrative costs of complying with the request or decline to comply with the request.
Colorado Users: You can make a request related to the Colorado Privacy Act (CPA) free of charge once within a 12-month period. For subsequent requests within a 12-month period, we may charge $0.25 per standard page, or a fee not to exceed the actual cost of providing a copy, printout, or photograph of the record in a format other than a standard page.
Connecticut Users: You can only make a request related to the Connecticut Data Protection Act (CTDPA) once within a 12-month period. This request will be free of charge. In cases, however, where the request is manifestly unfounded, excessive, or repetitive, we may charge a reasonable fee to cover the administrative costs of complying with the request or decline to comply with the request.
